A custom engagement ring is modeled in CAD, approved by you before production, and made by hand to the specification you signed off on.
What makes a ring custom
Three things: the stone is chosen specifically, the design is modeled rather than selected, and the ring is built to your measured size. Change any one of those and it becomes a made-to-order stock design, which is cheaper and faster but less flexible.

Designing around a specific stone
When you have a stone in mind, the setting gets designed around it rather than the other way round. This matters more than people expect. An elongated shape needs a setting that does not shorten it, a step cut needs a bezel or open gallery that does not fight its geometry, and a pointed shape needs protection. Budget guidance
The stone is usually the majority of the cost, followed by the metal and the complexity of the setting. If your budget is fixed, the fastest way to improve the result is to optimize the 4C's rather than to simplify the design. A well-cut 1.2ct in a clean setting beats a poorly cut 1.5ct in an elaborate one. See the 4C guide.
Custom is final sale
Because you approve the design before production, custom rings are final sale. Manufacturing defects are handled under warranty, which is a different matter entirely. See returns.
